Medical Stop Loss

• Binder Premium and Accounting Backup
• Disclosure Statement
• Final Medical Census as of the Effective Date of Coverage
• Satisfaction of contingencies from the quote
• Signed Application
• Signed Endorsements
• An approved Plan Document is required in order to reimburse adjudicated claims

Group Life

• Binder Premium and Accounting Backup
• Final Life Census as of the Effective Date of Coverage
• Satisfaction of contingencies from the quote
• Signed Life Application
